Camp Azamer 2024 Registration Form

I agree to adhere to all rules and regulations which govern: 1- my child’s stay at camp 2- payment schedules and terms. Camp Azamer reserves the right to dismiss without refund any child whose conduct is deemed detrimental to the camp in any way. No refund or reduction can be made for late arrival or early departure from the camp. The cost of replacement of any items damaged due to misuse or willful destruction by any child will be billed to the parents. I recognize that although Camp Azamer will operate with the highest level of safety there still may be potential hazards and risks. By enrolling my child I acknowledge that I recognize these risks. I also understand that Camp Azamer cannot be responsible for any items lost or damaged by fire or theft. Once camp is over I understand that all personal items will be disposed of.

I/we the undersigned, parent(s) of the Camper being registered in this form, a minor, do herby authorize Camp Azamer, as our agent to consent to any diagnostic procedure or medical care which is deemed advisable by and also to be rendered under the supervision of, any licensed physician/surgeon, when such diagnosis or treatment is rendered at a medical facility. It is understood that this authorization is given in advance of any specific need. It is understood that this authorization will be used in an emergency; all attempts will be made to contact the parents. The health history is correct as far as I/we know and the above mentioned child has permission to participate in all prescribed camp activities except as noted by us or our physician. In the event that I can not be reached in an emergency, I herby give permission to Camp Azamer to secure proper treatment for my child. This authorization shall remain effective until August 5, 2024.
