To complete your registration, please follow the steps below to make your deposit, complete the Medical and Authorization forms, and the Immunization form. 

Step 1: Submit your deposit

Click here to make your non-refundable $50/week deposit.
This deposit will be applied towards camp tuition and is required to complete the registration process.

Step 2: Complete the Medical Consent and Authorization Form

Please complete all highlighted fields in the form below. Once all forms have been filled, select the Click to Sign button at the bottom of the form. Once the Click to Sign button has been selected, you'll need to verify your email address. The registration will not be submitted until the email address verification is complete!

Please make sure you completed ALL field in the form above, and have clicked the Click to Sign button that appears at the bottom of the form (if you don’t see the Click to Sign button please verify you’ve completed all fields including the checkboxes). You will then have to confirm your signature via an automatic verification email that is sent to you. 

Step 3: Complete the Immunization Form

Download the required Immunization form and get it signed by the child's doctor. Please email the completed forms directly to campazamer@gmail.com.
